Cuyahoga DD is pleased to provide an array of school supports for eligible students.
To make a referral, school personnel should complete the Referral Form for Cuyahoga DD School Supports (below) on behalf of the student. (If you are a parent/guardian seeking help for your child, whether that be in the school, home or community, please call 216-736-2673.)
