Accessing Individual Support Plans (ISP) via the Provider Search Tool
Individual Support Plans (ISPs) are not mailed to providers. They are accessible through provider accounts in the Provider Search Tool. Please contact the Support Administrator if you cannot access ISPs via the Provider Search Tool.
Providers have 30 days to notify the Support Administrator in writing when authorization changes are needed. This timeframe corresponds to the DODD notification requirements for the Monthly Rate Calculator settings. There is no longer a separate form to request a plan revision. Requests can be submitted to Support Administrators via email.
