Providers must maintain evidence that they have provided services through service documentation. Documentation must be:

  • Stored in an accessible location;
  • Provided, upon request, for review or an audit; and
  • Stored for six years from the date of payment or until the outcome of an audit, whichever is longer.

Providers are required to complete training in documentation and billing within 60 days of being selected to provide services. This training can be found on DODD's MyLearning website or through NEON. (See Resources for more information on training.)

 

Forms

DODD does not require providers to use one specific form to document services, however, there are requirements regarding what information must be documented. The requirements are stated in Rules specific to services being provided. Click here to learn more about these Rules.
